Catchment area
Before filling out a registration form, check you are in our catchment area.
The practice area includes St Albans, London Colney, Colney Heath, Sandridge, and Smallford.
Anyone living within this area is welcome to apply to register as a patient.
We cannot/do not accept new patients from: Chiswell Green, Harpenden, Redbourn, Wheathampstead, Colney Street, Frogmore, Shenley or Bricket Wood.
If you wish to become a patient at Parkbury House or wish to return to us after a period of travel or absence please complete a registration form at reception or complete and submit a form using the link below.

Registering with the surgery
Patients can easily register with our surgery online by filling out the online form here.
Alternatively, the NHS also provides a new online service called ‘Register with a GP surgery‘ that makes it easy to register. Please click here to access this service.
Paper forms are still available from reception if you need one.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
For adults registering at the practice – we will require proof of address and photo ID.
For children registering at the practice – we will need a copy of their birth certificate and immunisation record from the red book.
Please be aware that we can’t complete your registration without seeing these documents.

If you don’t have proof of ID or proof of address – please speak with reception and we will help you to prove who you are and where you live.
Re-registering and temporary patients
If you are a returning patient e.g. student, please make reception aware or add a comment in the comment box on the re-registration form of this fact so you can be re-registered and your new notes linked to your old notes. This is important for both time and continuity of care.
Temporary registration is suitable for any patient wishing to access doctors services outside their own clinic. Temporary residents will not be candidates for home visits but are welcome to attain advice from our team of doctors.
Please note, it is not necessary to re-register as a patient if you do not need to be seen face-to-face by a clinician, if you are only living locally temporarily. Often, your current (permanent) GP practice can advise patients and send prescriptions to a local pharmacy over the phone.
You must provide your NHS number when registering with the practice. You can obtain this information from your previous GP surgery. To register with Parkbury House Surgery submit a registration form.
Once you have submitted your form, it will be passed onto the registration department. Please note, if there are any fields which have not been completed or your NHS number is not included, this may delay your registration with us.
New patient prescriptions
Please note that we follow local prescribing advice and it may be necessary to change new patients’ current medication to match what we are allowed to prescribe.
These guidelines are Hertfordshire wide so this may only apply to you if you have moved from another county.
