If you are aged 18 or over, please bring your Purple Folder to every appointment so we can refer to it if needed. If you do not have a Purple Folder and would like one, please contact the surgery or use the link below.

Annual health checks

We offer annual health checks to people over the age of 14 to chat with you about your health and wellbeing.

Your annual health check takes place in two appointments.

First appointment:

You will see Laura Khan, one of our nurses. She will:

  • Check your blood pressure and other routine observations.
  • Take your annual blood tests.

If you would like Emla numbing cream before your blood test, please contact the surgery in advance so we can prescribe it.

Second appointment:

A few days later, you will see Zara Allardyce, our Physician's Assistant. She will:

  • Complete your annual health check and action plan
  • Discuss your blood test results.
  • Talk through any health concerns or questions you may have.

We aim to book you with the same clinicians each year, but this is not always possible.

Most health checks take place at the surgery. If you have difficulty travelling to us, we can arrange a home visit instead.
